Complete Form BOE-502-D/Change in Ownership Statement, Death of Real Property Owner Complete Form Affidavit of Death of Trustee
Complete Form BOE-502-A/Preliminary Change of Ownership Statement
(Optional Claim for Reassessment Exclusion for Transfer Between Parent and Child)   

Notarize forms (if applicable)
Make copies of forms (to keep for your records)
Mail forms to Los Angeles County’ Assessor  Office at Registrar-Recorder/County Clerk Document Analysis and Recording Section P.O. Box 1250 Norwalk, CA 90651-1250

Include copy of Death Certificate
Include trust and all amendments (will need signature original if recording in person)

You can also complete the recording in-person at the Van Nuys Recording office.
